Appeals Plus is $49, once, per case. It's not a subscription: nothing renews or recurs, and no further charge follows for that case. It covers every appeal level for 12 months, including a second or third round if the case goes further.

What Plus unlocks: the full appeal letter (download and copy-as-text, every level and every regeneration), the physician-letter request PDF addressed to the treating physician, the filled CMS-1696 (Appointment of Representative) form, and every future letter as the case advances through the appeal levels.

The checkout option only appears once a real letter already exists for your case — CaseWhy Appeals checks that first, on purpose, so you never pay for something that doesn't exist yet.
