Look at the card in your wallet — it tells you almost everything you need to know.
A plastic card with a company name
Humana, UnitedHealthcare, Aetna, Cigna, and others — a plastic insurance-company card means Medicare Advantage. If a plan (not Original Medicare) denied a service, item, or bill, that's the pack to use.

The plain red, white, and blue card
No company name, just the red, white, and blue Medicare logo — that's Original Medicare. This is the card to use for a hospital discharge, a rehab or home health service ending, a change to observation status, or a denied line on a Medicare Summary Notice.

A separate drug plan card
If a pharmacy couldn't fill a prescription, or a separate written denial came from a stand-alone drug plan, that's Medicare Part D — whether or not you also have a Medicare Advantage or Original Medicare card.
